When you call Urology Associates, we will work with you to schedule an appointment with the physician of your choice. If that physician is not available, we will be happy to suggest another qualified partner of the practice for you to consider.

We are open Monday to Friday (excepting holidays) between 8:00 AM and 5:00 PM.
When you arrive at our office, please bring the following information:
- Patient History Form - Adult
- Patient History Form - Child/Pediatric
- List of current medications
- Any medical records or x-rays related to your condition
- Insurance card(s)
- Insurance referral if required by your plan
Should you need to cancel an appointment
Please telephone the office during business hours and allow at least one day's notice so that we can offer your appointment time to patients on our waiting list.
We recognize that your time is valuable, and we make every effort to run on time. Occasionally emergencies or patients require a little more time, and these cause scheduling delays beyond our control. We apologize if we keep you waiting.
