The Urology Associates Second Opinion Program is provided as a resource for consultative advice and urological treatment recommendations for patients. We strongly suggest that you carefully read the following terms and conditions. Because this service is accessible online, you agree to abide by our Legal Disclaimer, as well as these terms and conditions described below. These terms and conditions are subject to change at the discretion of Urology Associates without prior notice.
Stored Information
While visiting our sites, we will learn information about you. You should know that we analyze information from your website visits in order to improve the website performance and understand usage. We look at the IP address from which you access the Internet, the date and time, the Internet address of the website from which you linked directly to our site, the name of the file or the words you searched, and the browser used to access our site and other similar information. All information collected to include personal information is encrypted using SSL (secure sockets layer) technology.
Personally Provided Information
If you choose to provide us with personal information, we use that information only to respond to your request. We do not give, share, sell, or transfer any personal information to a third party unless required by law.

2nd Opinion Consultation
The consultation provided through the Urology Second Opinion site is different from the diagnostic and consultative services typically provided by a physician. Physicians providing this service will not have the benefit of information obtained by an in-person examination. By deciding to use this site, you acknowledge and agree that you are aware of this limitation and agree to assume the risk of this limitation.
Your participation in this program will be considered an indication of acceptance of:
- The diagnosis you will receive is limited and provisional;
- The second opinion is not intended to replace a full medical evaluation or a face-to-face visit with a physician;
- The physician has not completed a physical examination and information gathered during a physical examination isn’t available; and,
- All information sent has been provided by the individual or with the consent and knowledge of the patient and is considered accurate.
State Jurisdiction
This Agreement shall be governed by and construed in accordance with the laws of the State of Colorado without regard to its conflicts of laws rules. Any dispute shall be brought in the courts of Colorado.
